Mental Health + Wellbeing Survey

The purpose of this survey is to understand the mental health and wellbeing needs of the customers and support people that Rocky Bay staff support across Western Australia. We know that one in five people express concerns for their mental wellbeing at any time across Australia, and this statistic is higher for persons living with other health and disability needs and their carers.

We want to learn what your mental health and wellbeing needs are, to ensure we can develop the skills of our staff and organisations and their ability to best respond to these needs.

Participation in this survey is voluntary and anonymous.

We are inviting customers and support people accessing Rocky Bay services to complete a short survey. The survey will consist of several questions and will take no longer than 10 minutes to complete. Basic demographic data will also be collected.
The survey will be available from 24 Feburary  2022.

Based on the information we receive, Rocky Bay will be able to better understand the mental health and wellbeing needs our customers and support people, and their experience of accessing Rocky Bay services.

Rocky Bay will use this information in order to understand the needs of our customers and support people mental health and wellbeing, which will inform staff training and organisational development initiatives. We are responsible for looking after our customers information, using it and storing appropriately in a confidential and anonymous manner.

The survey does not include any questions that can identify the customer or support person either by name, e-mail address, telephone number, or other identification. The answers can thereby not be linked to any individuals, neither directly nor indirectly, unless the customer or a carer has chosen to provide this information.

The survey data will remain confidential between you and Rocky Bay. Any information stored that you provide will be anonymous and not identifiable to you.
