If you are a person living with disability who wants to live in your home for as long as possible, we provide you with the support to do so.

Our team of therapists, health and wellness officers, psychologists, social workers and nurses work together to provide services tailored to your needs, providing a long term approach to your quality of life and independence.

Services may include:

  • mobility and transfers
  • home environmental controls and smart technology
  • support with communication and decision making
  • guidelines to supporting your loved ones with dementia
  • falls prevention
  • functional rehabilitation and therapy
  • engagement and participation in meaningful activities

You can access our services if you have an NDIS or WA NDIS plan, Disability Services Commission funding, Better Start funding, Medicare or if you want to fund your own services. We can also support you through the planning process if you do not have a plan already.
