Social Work

Rocky Bay's Social Work service aims to support families and individuals to maximise their capacity for self-care and integration into the community, as well as improving the quality of life for clients by facilitating access to a range of available supports and services.
Social Work can assist with:

 

Practical Assistance

  • Providing information regarding benefits and entitlements, housing options and emergency assistance;
  • Providing information regarding carer respite services and in home domestic services;
  • Assistance liaising with government departments and other agencies such as Centrelink, Homeswest, hospitals and schools;
  • Referral to appropriate services eg Silver Chain, Local Area Coordinators;
  • Assisting with funding submissions eg Disability Services Commission, Lotterywest, local clubs and associations;
  • Providing information regarding available community services and resources;
  • Advocacy and consultation with other service providers eg schools, support agencies and hospitals.

 

Emotional Support

  • Information and support through initial diagnosis;
  • Confidential counselling and support around areas such as adjustment and change, loss and grief, self esteem, stress management and coping strategies, parenting strategies, relationship issues, interpersonal conflict (please note referral to specialist agencies may be required);
  • Working with families who are concerned about their child's behaviour;
  • Supporting parents to develop positive parenting strategies;
  • Linking to support groups for parents, siblings and partners.

 

Education and Transition Planning

  • Providing information to schools and other agencies about neuromuscular conditions to support client inclusion, acceptance and participation;
  • Supporting through significant life transitions eg from primary to high school, to independent living, school to work/Post School Options, from Children Services Team to Adult Services Team (refer Fact Sheet - Transition from Children Services to Adult Services).
