Community Nursing Services
Community Nurses provides resources, expertise, knowledge and support to people with disabilities living alone and independently in the community. Their focus is on the promotion and maintenance of health, early recognition of and prompt intervention in the disease process, supporting rehabilitation activities and the promotion of community integration, thereby assisting the person to achieve maximum independence and an optimal quality of life.
Rocky Bay's Community Nursing works within the Clinical Services Directorate as part of an interdisciplinary service, according to the Disability Services standards.
The Community Nurse is funded to provide a clinical nursing support service to adult clients who are registered to receive services from the Therapy and Professional Services (TAPS) team and Rocky Bay's Community Living Accommodation Support Service, in nominated community houses.
The Community Nurse is primarily responsible for:
- Assessing the clinical nursing needs of individuals;
- Providing direct clinical nursing advise/consult and support;
- Liaising on behalf of clients with external service providers across health and disability sectors;
- Providing education and follow-up to Disability Support workers to assist them to support Rocky Bay clients in nominated community houses;
- Providing a support and consultancy service for Rocky Bay staff as appropriate;
- Providing education and information as required for targeted Rocky Bay clients;
- Participating in team planning and coordination.
Subject to available resources, the Coummunity Nursing service accepts referrals to provide one-off consultations for other adult clients living in the community and registered for TAPS services. Clients may be referred to other appropriate community based services as appropriate.
All new requests for this service should be forwarded to the Manager TAPS.
