Community Home Service
Community Home Service is a service that supports people with a disability in their family home and/or in their own home. This service aims to assist the person maintain their independence and support the family.
- The Community Home Service is open for expressions of interest to any person with a disability who are funded recipients of an Intensive Family Support (IFS) grant or Accommodation Support Funding (ASF);
- Individuals interested in utilising Rocky Bay as a service provider can refer themselves as a potential client. An individual assessment of needs is conducted to determine whether Rocky Bay is the best organisation for them;
- During the assessment process the person with a disability puts forward how they would like their Intensive Family Support (IFS) hours utilised to best meet their individual lifestyle and needs. This service aims to be flexible and will tailor to the individual's and family need;
- Individuals who have access to Accommodation Support Funding (ASF) , Rocky Bay's Community Home Service, whilst limited, have opportunity for people to live independently in their own home and/or a shared option. The same process will apply where an assessment is conducted to determine whether Rocky Bay is the best organisation for them;.
- The Community Home Service support team have experience working with people with disabilities and promote an independent community lifestyle.
