Referral Criteria
- Referrals are accepted from any source.
Admission Criteria
- Rocky Bay is funded to provide services to persons who have neuromuscular or neurological disorders as their primary condition.
- Services are limited to the number of individuals specified in the Purchasing Agreement between Rocky Bay and the Disability Services Commission.
- Where resources allow Rocky Bay may offer services to persons who have needs similar to this primary population. Priority will be given to persons who cannot access an alternative service and where Rocky Bay services will maintain or improve a person's quality of life.
- Services may only be offered within the financial resources available through the Purchasing Agreement.
- Provided funding is available for specific services such as Employment, Post School Options, Respite, Peel School Age Therapy Services and Community Aids and Equipment Program, Rocky Bay will offer these services to people having other types of disabilities however priority will be given to Rocky Bay clients.
- Specific Referral/Admission/Exit Criteria for these funded programs are attached to this policy.
- Individuals with potentially challenging behaviour, who may overtly threaten the safety and wellbeing of others, will not be accepted for services.
Where prospective clients meet the above criteria but no vacancies exist within the organisation, clients are placed on a waiting list which is regularly reviewed by the Rocky Bay Council. Priority for service provision will be based on individual need.
Exit Criteria
Exit procedures will take place where :
- a client voluntarily elects to leave Rocky Bay's services;
- a client breaches behavioural expectations on more than three occasions. In such situations the Chief Executive Officer will decide whether that client can continue to receive services;
- changes in a client's medical circumstances necessitate transfer to another agency; or
- a client has not used Rocky Bay services for a two year period. Should their circumstances change they may be reconsidered for admission.
